Step Five: Continuous Improvement and Sustainability


After working diligently to make systems change, the Team will want to put into place structures, strategies, routines, or practices that ensure the changes stay in place, and evolve as conditions change. Sustainability of the outcomes and products of the Team’s efforts requires attention to a number of details, including sustaining the Core and Implementation Teams, continuing to gather and analyze progress monitoring data, and continually reviewing, revising, updating and adding goals and strategies to the plan.
