In this session, LaDonna Morales of Cincinnati Children’s Hospital Medical Center, will discuss how to prepare individuals and their families for the transition from pediatric medical care to adult healthcare. Join us to learn more about how to prepare individuals and their families on how to become an adult in the medical setting, how to engage and work with pediatric providers during this transition, how to decide who will be on the adult medical team, and how this all ties into transition planning for youth.
